How to Gather or Craft: Medicine for the knowhow, proper handling, and application. Science/pottery/glassblowing/smithing/textile production or any skill granting knowledge and manipulation of physical properties of base materials. Magic practices and magic domains alone aren't sufficient to craft a poison well.
Skill Requirements: Master Medicine, Expert secondary skill (see crafting information).

Uses: A single poison well can be used to spoil/poison/drug a large well's water supply. Otherwise, a single Poison Well can supply ten doses of highly toxic ingested poison in the short term. It can be converted to different delivery methods with mastery of medicine.

Information: A Poison Well is a catch-all term for a grab bag of various toxins that have been chemically bonded to each other through a process of artificial petrifaction, using a variety of methods not limited to heating in a stove, and crafting a ceramic or another material matrix of the toxic materials with various material bases.
Side effects/complications/weaknesses : Those drinking from a well tainted by a Poison Well will experience stomach upset first. Then their insides will begin to agitate if they fail to procure the services of an expert healer to provide an antidote. The tertiary effects depend upon the exact materials and chemicals used to craft the Poison Well. It could range to anything from hallucinogenic, to fatality inducing. Unfortunately, due to the nature of Poison Wells, more than one toxin is often used to craft them, and as such it's very difficult to know the exact formula. However, activated charcoal has been known to slow the effects long enough so that treatment can be found before its tertiary effects take hold.
